Pharmacuetical Care - New oppurtunity for Pharmacist
In 1990 Hepler and Strand defined pharmaceutical care as:
“Pharmaceutical care is the responsible provision of drug therapy for the purpose of achieving definite outcomes that improves a patient’s quality of life”.
In 1998 FIP adopted this definition with one significant change as:
“Improve or maintain a patient’s quality of life”
Pharmaceutical care is team work of physicians, nurses, pharmacist including patients. It is cycle where pharmacist asks questions to patient related to problems. According to answers pharmacist identify the drug therapy problems. Next he/ she develops care plan to resolve or prevent the drug therapy problems. Here individual patients need is different as problems are different for each patient. Developing care plan means setting therapeutic goal to achieve required clinical outcome. After this implementation and monitoring of care plan is done by pharmacist. It is cycle cause each time during visit pharmacist asks questions to determine if problem has been resolved or if a new problem has arisen.
Categories of drug therapy problems that pharmacist should identify before setting therapeutic goal for the treatment, as per WHO are:
1. Appropriate indication for the medication
* Needing drug therapy but not receiving it * Receiving unnecessary drug therapy
2. The most effective medication
* Receiving the wrong medicine * Dosage too low
3. The safest medication
* Too much of the correct drug * Adverse drug reactions
4. Patient adherence and convenience
Pharmaceutical care mainly focuses on individual patients and sometimes population too. “Population-based pharmaceutical care” uses demographic and epidemiological data to establish formularies or medicine lists, develop and monitor pharmacy policies, develop and manage pharmacy networks, prepare and analyze reports of drug utilization/costs, conduct drug utilization reviews and educate providers on medicine policies and procedures.
Individualized pharmaceutical care is patient centered practice as pharmacist who is responsible person has to identify & resolve or prevent the drug therapy problems for each patient.
The pharmaceutical care practice helps to achieve optimal therapeutic outcome of therapy. It instrumental in identification and prevention of adverse drug reactions and drug interactions. It is achieved through effective patient counseling and documenting patient’s medication (prescription drugs and nonprescription drugs), family and social history. The documentation is important aspect for management of therapy, any adverse drug reaction or any allergic condition to the patient is identified at the earliest as it facilitates further treatment of the patient. The knowledge support by clinical pharmacokinetics will add on value to the therapeutic outcomes .The clinical pharmacist takes active role in therapeutic drug monitoring of narrow therapeutic index drugs. The Clinical pharmacists are also supposed to attend medical rounds and give all necessary information to physicians, nurses and patients regarding drugs usage along with the cost analysis which directly reduce burden of therapy cost.

Our society and our health care sysytem
May be it took 5 decades to establish pharmacuetical care in those countries where the role of pharmacist is well established in the clinics. Here also in some of the hospitals especially in corporate hospitals pharmacists are consulted for the pharmacuetical knowledge.
It took many years to establish the clincial pharmacy over there. Here also there will be situations and conditions which demand the services of clinical pharmacist, community pharmacist and hospital pharmacist, at that time like what and how it happened in developed countires , similarly changes will take place,
For example, with the blessings and permission of Central govt, has introduced the Pharm D degree education program . These pharmacist will have exposure to clinical pharmacy.
with in three years first batch of Pharm D Berreculate are avialble for service,
what a change, it happened sileantly,
The acceptance of the Pharm D as a staff of hospital staff wiil be there , just wait and see

Over the time scale it is not only the pharmacuetical care almost all the developments take time to get accepted in a tradational society like ours.We Indians have our own concept like to care , protect the pharmacuetical industry hence our Government is intrested in policies to have health care polices that are product oriented than patient oriented. Pharmacuetical care is the mode of practice which is in its infancy.There is a need for all of us ( we the Pharmacists) to nurture and make it grow as fast as possible.
There is a demand for services where in patients are getting benifited you are right it is not very widely practised.

Opppurtunites and remuneration
Dear Faria,
Pharmacuetical care is a professional oppurtunity to establish and divert the product oriented society to the patient oriented society.
Now the fear among the youngster is about thier future in the field.
Just tell me how do we justify the industry claim that consume more medicine to stay healthy, that how the pharmacuetical care has grown over in developed countires..
It is very important to think about patients who are near and dear to us to protect from unnessary exposure to drugs, irrational use of medicine,
these are the issues challenging every one of us, we the pharmacisits should volunteer for the cuase. the oppurtunites and remuneration will just fallow,
