With a series of monologue blogs, lets have a dialogue blog for a change. In this blog, the reader ought to give the name of a drug that must start with the letter that had been the ending of the word used by the last reader. And yes, the 'word' must only be a drug name; generic or trade name. Additionally, the category of the drug and its mechanism of action could be put in brief. Although, this might seem like a game, but it would really educate us about new drugs and their details which we have been unaware of. Moreover, it would also help one to educate the other readers. So, starting with an inaugural drug:

Edrophonium (generic name)

Category-

Anti-cholinesterase or cholinesterase inhibitor (reversible type)

Mechanism of Action (MOI)-

It works by binding to the active site of enzyme cholinesterase reversibly by hydrogen bonds and serve to deactivate the enzyme, thereby preventing metabolism of acetylcholine in the body and raising its level leading to parasympatomimetic actions.

Use-

Its use is prophylactically limited mainly to diagnose myasthenia gravis.

RALTITREXED

Category - Antineoplastic

Mechanism of action - Raltitrexed is chemically similar to folic acid and is in the class of chemotherapy drugs called folate antimetabolites. It works by inhibiting thymidylate synthase (TS), an enzyme used in pyrimidine synthesis. Raltitrexed is fully active after polyglutamylation. By inhibiting the formation of precursor pyrimidine nucleotides, raltitrexed prevents the formation of DNA and RNA, which are required for the growth and survival of both normal cells and cancer cells.

Category - Class II Antiarrhythmic agent
Mechanism Of Action:-
Class II agents are beta adrenergic antagonists. this drugs diminished Phase 4 depolarization thus depressing automaticity, prolonging AV conduction, and decreasing heart rate and contractility.Class II agents are useful in treating tachyarrhythmias caused by increased sympathetic avtivity.

METFORMIN
Category: Anti-diabetic
Mechanism of Action:
It acts by reducing Gluconeogenesis and thereby reducing the overall hepatic production of glucose. It also acts by enhancing the activity of AMP activated protein kinases that incolved in protein, carbohydrate and fat metabolism.
On overall it increases glucose uptake by GLUT transporters, decreases gluco production, minimizes fatty acid oxidation etc.

So the next drug shoul start with "N"

Category- anti-hypertensive

Mechanism of action- It is a calcium channel blocker... Blocks the influx of calcium ions needed for contractility of heart muscles...
