SITAGLIPTIN...Controversy vs Explanation

Recently FDA notified healthcare professionals that Januvia (sitagliptin) and Janumet (sitagliptin/metformin) are known to cause severe pancreatitis as on Sep 25th. Following this alert, Merck responded in the following way..
"Merck has thoroughly reviewed the safety data for sitagliptin, and sitagliptin was not associated with an increase in the incidence of pancreatitis in preclinical studies or in clinical trials of up to two years in duration with more than 6,000 patients. Merck has also carefully reviewed post marketing adverse experience reports, and Merck believes these data do not demonstrate that a causal relationship exists between sitagliptin and pancreatitis. Merck appreciates the important role that the FDA plays in assessing the safety of medicines and, as we do with all of our medicines, we will continue to monitor the safety of JANUVIA and will share the data with regulatory agencies and the medical community," said John Amatruda, M.D., Senior Vice President and Franchise Head, Diabetes and Obesity, Merck Research Laboratories.
"Patients with type 2 diabetes are more likely to develop pancreatitis than other people" he added.
I was surprised to see that both the alerts ie. reg usage of sitagliptin and Merck responding to the alert happened to be on the same date.

The notification & Mercks reply to it, is not exactly as you have put it.

According to AERS between Oct 2006 to Feb 2009, there were 88 cases of acute pancreatitis in patients taking Sitagliptine & sitagliptine/Metformin.

(The analysis found that 19 of the 88 reported cases (21%) of pancreatitis occurred within 30 days of starting sitagliptin or sitagliptin / metformin. Furthermore, 47 of the 88 cases (53%) resolved once sitagliptin was discontinued. It is important to note that 45 cases (51%) were associated with at least one other risk factor for developing pancreatitis, such as diabetes, obesity, high cholesterol and/or high triglycerides. Based on the teporal relationship of Initiating sitagliptin or sitagliptin / metformin and development of acute pancreatitis in the reviewed cases, FDA believes there may be an association between these events.

Based on these reports, FDA is working with the manufacturer of sitagliptin and sitagliptin / metformin to revise the prescribing information to include:

1) information regarding post marketing reports.
2) Information that - regular monitoring of patients on sitagliptine therapy to avoid development of pancreatitis.
3) Information noting that sitagliptin has not been studied in patients with a history of pancreatitis.
