ABSORPTION PHASE:

Absorption nay be altered due to alteration in any of the four mechanisms
1. Gastrointestinal motility and gastric emptying rate.
2. Gastrointestinal flora.
3. pH of the GIT.
4. Free drug available in GIT.

Alteration in Gastric motility and Gastric emptying rate:
GI absorption is slowed by drugs that inhibit gastric emptying such as atropine
or opiates or accelerated by drugs such as metoclopramide

Alteration of pH:
Antacids, Sodium bicarbonate, Calcium carbonate may alter the pH of the GIT
and thus alter the absorption of drugs whose absorption is dependant on pH such
as Iron products, aspirin, NSAIDS.

Alteration of Gastro intestinal Flora:
Some antibiotics destroy the gastro intestinal flora and thus prevent the absorption
of some other drugs.

Availability of free drug:
Calcium and iron form insoluble complex with tetracycline and retards its absorption

Antacids containing Al, Mg and Ca and sucralfate cholestryamine.

Formation of poorly sonluble and unabsorbable complex with such heavy
metal ions.

Reduced absorption due to complexation with metal ions.

Reduiced absorption due to absorption and binding.

Alteration of GI pH:

Sulfonamides, Aspirin, Ferrous sulfate, Ketoconazole, Tetracycline, Tetracycline,
Altenolol

Antacids, Sodium bicarbonate, Calcium Carbonate

Enhanced dissolution and absorption rate.

Decreased dissolution and hence absorption

Decreased dissolution and bioavailability

Alteration of Gut Motility :

Object dugs

Aspirin, Diazepam, Levodopa, Lithium carbonate, Paracetemol,
Mexiletine
Metoclopramide, Anti-cholinergics (atropine, Homatropine)

Rapid gastric emptying

Increased rate of absorption

Delayed gastric emptying

Decreased rate of absorption

 

Alteration of microbial flora :

Object dugs
Precipitant drugs
Influence of object drugs

Digoxin

 

Oral Contraceptives

Antibiotics (Erythromycin, Tetracycline)

 

Antibiotics (Ampicillin)

Increased bioavailability due to destruction of bacterial flora that
inactivates digoxin in lower intestine.

Decreased reabsorption of drugs secreted as conjugates via bile in the
intestine.

Malabsorption Syndrome:

Object dugs
Precipitant drugs
Influence of object drugs
Vitamin A, B12, Digoxin Neomycin (and Colchicines) Inhibition of absorption due to malabsorption/ stearorrhea caused by neomycin.

A good blog...in the beginning of the blog you have mentioned the 4 factors that affects the absorption in human body......can you tell the factor which contributes maximum to absortion...

Dear zarrin,
Thank you for your valuable comment. It is the first factor i.e the gastric motility and the gastric emptying rate that contributes maximum to absorption. And the presence of villi increases absorption much more on account of their enormous surface area. It is important to note that doctors give pre and post prondial advice for every medication because of this reason.
